Therapy seemed to be most effective in preventing delirium tremens in patients without a prior history of delirium tremens. Of those preimplementation admissions without a history of delirium tremens, 16% had an episode during the admission. Only 2.6% of postimplementation admissions with no history of delirium tremens had a current episode. However, of patients with a history of previous delirium tremens, 36.8% and 40.0% in the respective cohorts had an episode during the current admission. Basis of disease. In the arena of prostate cancer research, an immediate goal is to determine whether widespread prostate specific antigen PSA ; screening is effective in the reduction of prostate cancer morbidity and mortality, so as to establish effective guidelines and begin implementing them worldwide. A second priority is to develop effective treatments for metastatic, hormone-refractory disease. As with prostate and breast cancer, a tremendous number of products are under development for lung cancer see Exhibit 4 ; . Beyond the possibility of these products, an immediate goal for improving survival rates for lung cancer is the design and implementation of national screening programs for early detection of disease. Methods: Five patients received total 10 treatments of MARS due to hyperbilirubinemia, in which 600ml of 20% human albumin was used as circulating dialysate and regenerated by two adsorbents columns built-in the circuit. At the 0, 3rd hr, 6th hr and 8th hr the end ; of MARS therapy, the concentration of total, direct, indirect bilirubin TB, DB, IDB ; and albumin in serum and albumin dialysate were measured. Besides standard MARS therapy, a modified treatment was performed in one patient, in which the two adsorbents columns were bypassed in the first 4-hr term, until in the second 4-hr term, they were then connected into the circuit and standard MARS was performed. Results: The reduction rate of TB, DB and IDB was 26.69.0%C29.59.6% and 14.812.3%, respectively. The concentration of bilirubin in albumin dialysate was between 21.622.5umol L. The molar ratio of TB to albumin in serum and dialysate varied during MARS, and existed great gaps at the end of the treatments. For the modified treatment, during the first 4 hrs, in which only albumin dialysis was performed, the TB concentration, molar ratio of TB to albumin in albumin dialysate elevated markedly, and the latter increased from 0.03 to 0.15, comparable with the value in serum 0.26 ; . During the second 4 hrs, which standard MARS was performed, the TB concentration and molar ratio decreased rapidly. Trospium has a quarternary ammonium group as opposed to the tertiary ammonium group of atropine ; that prevents it from crossing the blood-brain barrier into the central nervous system. Trospium is excreted in the urine mostly as the parent compound and due to its low rate of dry mouth is well tolerated. Two developmental-stage products that stand out are solifenacin Vesicare, Yamanouchi GlaxoSmithKline ; and darifenacin Enablex, Novartis ; . GlaxoSmithKline and Yamanouchi's drug, Vesicare, and Novartis's drug, Enablex, have both received an "approvable letter" from the FDA. Both of the companies are required to submit additional information to the FDA and these drugs are not expected to reach the market until 2H 2004. Solifenacin is available as a 5mg and 10mg dose that has only been tested in Europe. The 5mg dose has an incidence of dry mouth believed to be in the high single digits, which will make it an extremely competitive drug for Trospium. In addition, the marketing power of Yamanouchi and GlaxoSmithKline will make the marketplace challenging for Indevus. The 10mg dose has a 23% incidence of dry mouth incidence and darifenacin has a comparable incidence rate to solifenacin. Assessment All compounds are antimuscarinics with varying specificities for the muscarinic receptor subtypes. All antimuscarinics appear to have similar efficacies in treating the symptoms associated with overactive bladder; however, the most predominant issue with the use of antimuscarinics is the adverse reactions, particularly dry mouth. The M3 muscarinic receptor also regulates the salivary glands; consequently, inhibition of the M3 receptor results in decreased salivary secretion and dry mouth. Thus, an antimuscarinic specific for the M3 receptor is not necessarily as desirable as a nonspecific antimuscarinic with better tolerability: this is the case with current antimuscarinics. Studies have shown that dry mouth is reported in 6980% of patients taking oxybutynin, which is more selective for the M3 receptor, compared to 30-50% of patients taking tolterodine, a nonselective antimuscarinic. In contrast, a meta-analysis of clinical trials performed with Trospium, a nonspecific antimuscarinic, reported 4.1% of patients taking Trospium developed dry mouth. Although there are no published clinical trials with darifenacin, a pre-clinical study on guinea pig detrusor muscle has shown that, although darifenacin exhibits selectivity in binding for the M3 receptor, its effect on bladder contraction and salivary glands are equivalent. Solifenacin had a fairly equivalent effect on bladder contraction and inhibiting salivary gland secretion. The study did not demonstrate any beneficial effect of cognitive psychotherapy on DU disease as measured by recurrence rate during a 1-year observation period. Quite to the contrary, our results showed that this form of therapy might reduce the relapse-free time unless the patient was given supportive acid-reducing medication during the initial phase of therapy 18 ; . However, when the patients were given acid-reducing medication, there were no differences in occurrence of, or latency to, the relapse between the therapy group and the control group. Our results are in contrast to previous reports that have shown beneficial effects of psychotherapy on relapse rate in DU disease. However, all of these studies had small numbers 9-11 ; , or the results were not controlled by endoscopy 12 ; . Chappell and Stevenson 9 ; gave group lectures to 32 patients, and 20 control persons received routine medical treatment. Allocation to treatment was not randomized; there was a high dropout rate and incomplete follow-up. Orgel 10 ; gave complete psychoanalysis to 10 patients, and 5 drop-outs served as a control group. Brooks and Richardson 11 ; gave 11 patients 8 sessions of individual emotional skills training during a 2-week period; the 11 control persons received passive attention. The result was encouraging in favor of psychotherapy, but the follow-up period was short 60 days ; , and relapses were not controlled by endoscopy, which is necessary to confirm the diagnosis and to detect asymptomatic relapses. Lack of endoscopy is also the main weakness of Sj0din's 12 ; otherwise well-designed study of 103 patients. His finding that psychotherapy can improve dyspeptic symptoms was partly supported by our results. The only other study in which the result was controlled by endoscopy 14 ; showed a negative outcome of psychological counselling on the relapse rate, a finding perfectly compatible with our results.
